When might a doctor's note be required?

The National Agency for Conscription and Assessment requests information regarding, among other things, mental health, previous contacts with healthcare providers, diagnoses, and treatments. In some cases, you may need to supplement your application with a medical certificate from a psychiatrist.

• ADHD or ADD
• autism spectrum disorder
• Tourette's Syndrome
• other neuropsychiatric diagnosis
• past or current mental illness or psychological distress
• previous psychiatric treatment or medication

If we can help you, we’ll schedule an appointment for a virtual visit with a psychiatrist.

Visit us before your doctor's appointment
Before your virtual doctor’s appointment, you’ll need to visit our office to verify your identity, fill out a health declaration, and complete make your payment.

Digital Psychiatric Consultation
You will meet with the psychiatrist online. During the consultation, the psychiatrist will review your current and past mental health. The assessment is based on the consultation and relevant medical records.

Medical Certificates Are Issued
Once the assessment is complete and sufficient documentation is available, a medical certificate can be issued based on the specific issue at hand.

Objective Medical Assessment

Our medical certificates are based on an individualized and objective medical assessment. The psychiatrist makes a determination based on what emerges during the consultation and the available medical documentation.

A doctor’s note does not guarantee a spot, admission, or approval by the National Service and Examination Board. The note may be used as medical evidence in their final assessment. The National Service and Examination Board makes the final decision in your case.
